Tax Exemption

If you would like to apply for tax exempt status please upload one or more documents for review. 

Please Sign your Certificate, even if your certificate does not contain a section for a signature, we do require one. For fast approval, certificates should only be submitted with a clearly visible signature.

Please note the following:

• Out-of-date certificates or unsigned certificates will be rejected
• Certificates without "Vendor" section filled in will be rejected, please notate vendor as "OptiMA, Inc."
• A tax certificate must be included for each delivery state address
• Please ensure that your account profile is up-to-date and the company name on your account matches the company name on the certificate. To update your account profile, start by creating an account or edit your account settings
• Once approved, you will be able to place tax exempt orders on this website

If you would like to have tax removed from an order, please upload exemption documentation per state, or blanket exemption certificates for review. Tax exemption applies to orders based on the ship to destination.
